What we notice
People arrive with an injury that came from training hard while sleeping five hours and eating too little for the load. The training was not the problem. Everything around it was, and that is a more useful conversation than the one about the exercise that hurt.
The scale is a poor instrument
Body weight moves with fluid, food volume, hormonal cycle and glycogen, and can vary by two or three kilograms across a week with no change in body composition at all. Someone gaining muscle and losing fat can see nothing move for months. It measures something real and it measures it badly.
What to do instead
Take a small number of measurements that reflect function — what you can lift, how far you can walk or run comfortably, how you sleep, how your clothes fit — and review them every few months rather than daily. Slower feedback is less satisfying and far more accurate.
When it stops being healthy
Preoccupation with appearance that dictates social plans, produces guilt around eating, or continues through injury and illness has moved past health. That is worth taking seriously and worth speaking to someone about, and it is far more common in otherwise very fit people than most assume.
